Quelles sont les stratégies mises en place par les médecins traitants pour répondre à la demande de nouveaux patients ? Étude qualitative réalisée en Isère et Haute-Savoie

Abstract : Introduction: The concept of attending physician appeared in 2004 with the law on health insurance. His missions are numerous and described in the Public Health Law. He plays an essential role in the patient care pathway. However, for many years, medical demography continues to decline. At the same time, the need for care increases with the aging of the population. These phenomena would contribute to the scarcity of the supply of first-aid outpatient care. Objective : Identify the strategies implemented by attending physicians to meet the demand for new patients. Methodology : Qualitative study by collective and individual semi directed interviews carried out with twenty general practitioners from Isere and Haute-Savoie. Informations were collected until data saturation was reached. The interviews were transcribed and analyzed using the grounded theorizing method. Results : To meet the demand of new patients, practitioners have adapted their work organization. The presence of a secretariat allowed better management of the schedule with a barrier role towards the many requests from patients. Practitioners have established criteria to regulate their patients such as the geographical area, the age of the patients, the family or home visits. They also advocated better coordination of care through better communication between city medicine and hospital, but also with practitioners in the same area. Finally, the importance of the quality of life of the practitioner was approached with the objective of a good balance between private et work life. Conclusion : Physicians promote group exercise to reduce workloads. They recommend the delegation of tasks via secretariat, nurses and others, which could free up medical time and best meet the demand for care of the population.
