Création d’une consultation dédiée à la déprescription chez les patients âgés de plus de 65 ans en médecine générale : quelles modalités ?

Abstract : Deprescribing can be defined as a process to taper, reduce or stop medications which are considered inappropriate in an individual. It is a public health issue and need to be used everyday. The aim of this thesis is to create a deprescribing consultation model in general practice. A Delphi method was used to create a consensus by questionning an expert panel through three survey rounds. Thirty-eigh doctors participated, included general practionner, internist and geriatrician. The Delphi process idientified the mainlines of deprescribing : all patients are concerned, eigh drug classes should be reevaluated first including psychotropic, the time between deprescrinbing and its assessment must be related to the drug’s pharmacocinetic and collaboration between health professionals is essential. However none support for deprescribing have been highlighted. The results suggest that evidence-based deprescribing guidelines are a priority.
