Prise en charge de l’onychocryptose (ongle incarné) par les médecins généralistes bretons : étude transversale descriptive

Abstract : Aims: How do Breton general practitioners manage onychocryptosis in ambulatory medicine? Methods: Quantitative cross-sectional study. The collection was carried out by survey of 9 questions sent to Breton general practitioners from 21/11/2019 to 10/02/2020. Results: 257 answers have been analyzed. The characteristics of the population were: 55,25 % of women, 51,36 % under 40 years old, 94 % having an only outpatient activity, 66,93 % physicians practicing in Ille-et-Vilaine and 44,8 % in semi-rural area. At mild stage, we found: 44,35 % prescription of antalgic drugs, 91,05 % prescription of conservative therapies, 12,5 % prescription of local antibiotics, 0,39 % prescription of oral antibiotics. 91,05 % corrected risk factors.1,95 % performed the partial matricectomy. 29,57 % addressed to podiatrists.1,17 % addressed to medical specialists. At severe stage, we found: 71,98 % prescription of antalgic drugs, 59,63 % prescription of conservative therapies,14,4 % prescription of local antibiotics, 39,3 % prescription of oral antibiotics. 63,4 % corrected risk factors. 7,39 % was performing the partial matricectomy. 27,63 % addressed to podiatrists. 62,26 % addressed to medical specialists. The main obstacle to the partial matricectomy was the lack of training (80.93 %). 4 physicians performed this surgery during a dedicated training, 14 with a colleague specialist, 9 with a general practitioner. 11 were trained by their own means. Conclusions: More than 3/4 of general practitioners considered than partial matricectomy was in the area of competence. However, few practitioners performed this surgery due to lack of training. The development of a manage protocol and the creation of practical workshops could correct this deficiency.
