Validation du système de codage des quatre habitudes (SCQH) pour l’évaluation des compétences relationnelles en médecine générale ambulatoire

Abstract : Objectives - Physicians' interpersonal skills are essential non technical skills in the practice of primary care. Evaluating and understanding these skills is an issue for improving the quality of care. The French version of the 4-Habits Coding Scheme (4HCS) is a standardized tool for measuring these skills in medicine. The main objective of this study was to validate this scale within general outpatient medicine. Secondly, the study described this level of competence among general practitioners according to their level of experience, their gender, the duration of their consultation as well as the follow-up and to compare them to other care settings. Methods - The study was a cross-sectional observational study including outpatient general practitioners in Isère, Gard, Savoie and Haute-Savoie, as well as patients consulting their treating physicians. To allow the assessment of interpersonal skills by the 4HCS scale, the consultations were recorded and then analyzed by 2 investigators. A total score for each item was calculated. The validity, internai consistency and reliability of the scale was evaluated . Results - The mean total item score was 84.1 1115 (standard deviation: 16.6). The Cronbach's Alpha representing the internai consistency of the study was excellent at 0.953 [95% Cl: 0.93, 0.96]. The correlation between items was on average 0.69 (standard deviation: 0.13). The reproducibility of the measurement was good with an intra-class correlation coefficient of 0.82 [95% Cl: 0.74; 0.88] and 0.99 [95% Cl: 0.97; 0.99] representing respectively inter- and intra-rater reliability. The level of competence increased with the duration of the consultation, if the doctor was a woman, with little experience and if the patient was a primary consultant. Conclusion - The 4HCS scale is valid for the assessment of interpersonal skills in general outpatient medicine and can be transposed in this context for the purposes of assessment and improvement of skills.
