Dans l’étude SPICES, quels sont les freins à l’évaluation du risque cardiovasculaire par le score Interheart selon les évaluateurs ?

Abstract : Cardiovascular disease (CVD) develops gradually. Their appearance can be limited by controlling modifiable risk factors. The usual scientific scores include biological measurements that are expensive and require trained personnel. The SPICES project is an international, multi-center implementation study that evaluates the impact of non-pharmaceutical strategies in the onset of CVD using the Non-Laboratory Interheart Score (NLIHRS). This thesis conducted in Centre Ouest Bretagne (COB), seeks the elements which prevent the realization of the NL-IHRS when it is carried out by people who do not have full medical training. Method: it is a qualitative study using semi-structured individual interviews. Beginning students of nursing and medicine studies, final students of physiotherapy and citizen volunteers have conducted cardiovascular risk assessment events in the COB country. Subsequently, they were recruited according to sampling criteria including gender, age, education, place of residence and occupation of parents. The interview grid approached six themes; the interviews were continued until the theoretical data saturation. The results were analyzed by two researchers and the work was split into two theses. Results: the theoretical saturation of the data was obtained after nine interviews and confirmed by two additional interviews. The interview guide has been amended twice. Four main areas were identified: obstacles linked to the population, obstacles linked to the organization, obstacles linked to the evaluators and obstacles linked to the support available before and during the evaluation events. Conclusion: the media coverage around the evaluation of cardiovascular risk seems essential. In addition, health education seems necessary to raise awareness among the population. The risk assessment can be performed by non-medical personnel, but it is necessary to achieve a minimum training including the basics of the pathology concerned and also the basics of medical communication.
