Mise en place d’une filière de soins des diabétiques de type 2 au sein de la CPTS presqu’îles en mer après évaluation des attentes des différents professionnels de santé

Abstract : Background : General practitioners and nurses are on the front line to ensure the continuity of diabetic patient care. Diabetic care is a real public health issue. The objective is to limit complications and therefore reduce morbidity and mortality. Probably because of their geographical isolation, the health community “Presqu'îles en mer” testify of difficulties to access good healthcare and follow up when diabetes is diagnosed. Objective : Assess the shortcomings felt by general practitioners and nurses of the health professionnals of the community as well as their expectations regarding the care of type 2 diabetic patients. Propose a care networks for the diabetic patients care. Method : Descriptive qualitative study through semi-structured interviews with general practitioners and nurses practicing on the territory covered by the health community. Results : Between February and July 2020, 21 interviews were carried out. The main difficulties mentioned were “adherence to and compliance with the treatment”, as well as “difficult follow-up of recommendations” and “difficult management of complications due to geographic isolation”. According to the health professionals, patients report a difficult experience with the disease and a certain lack of information, especially on nutritional support and adapted physical activity. The professionals interviewed would like courses on reviewing the pathology and its management and easier access to specialized care. Several areas of improvement have been proposed and should be discussed in the work groups to judge their relevance and feasibility. Conclusion : To strengthen the therapeutic possibilities currently available, a specific care network led by a working group seems to have the support of professionals. A follow-up binder was created and offered as a support to facilitate patient follow-up, available to all caregivers. In the future, this binder could be digitalized to improve its ease of access.
