Résumé

Oncology implies the notion of end of life, accompanying the patient towards end of life care and confronting the use of medications realistically, in a health system with budgetary constraints and difficult decisions. The inventory of drug therapies available in oncology and used towards the end of life and the estimation of their cost is an important step contributing to the optimization of the management of patients at the end of life. A descriptive retrospective survey was carried out in 2018 at the Grenoble Mutualist Hospital Group, gathering the use of drug therapies and their costs during the last three months of life prior to death, with a focus on the last month of life. The secondary objectives were to expose potentially inappropriate drug use, to assess whether recommendations had been followed and to evaluate the use of a palliative approach. Of the 228 records analyzed, 44% of patients received injectable chemotherapy in the last three months, 23% in the last month. The overall cost of the cytotoxics administered during the study period was €170,347, 97% of which concerned expensive injectable molecules. The use of cytotoxics in relation to their performance score deviated from the recommendations of the learned societies. In oncology, Evidence-Based Medicine is confronted with the human component when making end-of-life decisions, and concerns both the patient and the healthcare professional(s). The participation of a pharmacist in multi-professional decisions would make it easier to guarantee the proper use of drug therapies.
L'oncologie implique la notion de fin de vie, l'accompagnement du patient vers le soin de support et confronte l’usage du médicament à la réalité humaine, dans un système de santé ayant des contraintes financières. L’état des lieux des thérapeutiques médicamenteuses utilisées en fin de vie en oncologie ainsi que l’estimation de leur coût représente une étape contribuant à l’optimisation de la prise en charge des patients en fin de vie. Une enquête rétrospective descriptive a été réalisée sur l’année 2018 au Groupe hospitalier Mutualiste de Grenoble, décrivant l’utilisation des thérapeutiques médicamenteuses et de leur coût au cours des trois derniers mois de vie précédant le décès avec un focus sur le dernier mois de vie. Les objectifs secondaires ont été d’évaluer les traitements potentiellement inappropriés, le suivi des recommandations et le recours à la démarche palliative. Parmi les 228 dossiers analysés, 44% des patients ont reçu une chimiothérapie injectable dans les trois derniers mois, 23% dans le dernier mois. Le coût global des cytotoxiques administrés pendant la durée de l’étude s’élève à 170347 € dont 97% lié aux molécules onéreuses injectables. L’utilisation des cytotoxiques rapportée au score de performance s’écarte des recommandations des sociétés savantes. En oncologie, l’Evidence-Based Medicine se confronte à la composante humaine lors des décisions entourant la fin de vie ; son accompagnement concerne autant le patient que le professionnel de santé. La participation d’un pharmacien aux décisions pluri-professionnelles permettrait de garantir le bon usage des thérapeutiques médicamenteuses.
