Étude de l’efficacité et de la tolérance de la stimulation transcrânienne à courant continu dans le traitement de la dépression

Abstract : Depression is the leading cause of morbidity and disability worldwide. Twenty to 30% of depressions fail to reach remission after two trials of antidepressant treatment. Transcranial direct current stimulation (tDCS) could constitute an interesting alternative to the management of this pathology.
MATERIAL AND METHODS :
The data of this study come from the DISCO study (cost-utility study of tDCS in the management of depression) at the Center Hospitalier du Rouvray. Its objectives were to study the efficacy and safety of a course of tDCS in uni or bipolar patients with moderate depression who have failed to one or two lines of treatment for the current episode. Patients were randomized to a tDCS arm or to a “usual management” arm (consisting in our study of repetitive transcranial magnetic stimulation, rTMS). Our primary aim was the rate of patients responding to tDCS at M1 (defined as a 50% decrease in baseline score on the Montgomery-Åsberg Depression Rating Scale, MADRS).
RESULTS :
Response rate was 50% in tDCS with a median reduction of MADRS score of 14.5 points (p =0.03552 et IC95% [5 : 16,5]). No serious side effects have been reported.
DISCUSSION AND CONCLUSION :
Our study has shown that tDCS could be an effective and very well tolerated treatment for moderate depression resistant to one or two treatments. Given its ease of use, it could be indicated as a secondline treatment for moderate depression.
