Efficacité de la Stimulation Électrique Fonctionnelle lors de la marche dans le cas du pied tombant chez le patient hémiplégique à la suite d’un AVC

Abstract : Background: Foot drop is a common condition in hemiplegic patients after stroke. It can lead gait impairment which can affect the patients’ daily life. The ankle foot orthosis is the therapy that is the most often used in that event. However, Functional Electrical Stimulation (FES) is one of the possibilities of the therapeutic arsenal used by physiotherapists. This therapy was discovered decades ago, it is still little used and still subject to controversy. Objective : The aim of this review is to assess the effectiveness of FES during gait associated with conventional rehabilitation in hemiplegic patients after stroke. Methods: After the investigation of several databases, four randomized clinical trials were included in this review. The control group received conventional rehabilitation after stroke, while the experimental group was treated by conventional rehabilitation associated with FES. Speed gait was used as primary outcome and assessed by 10-Meter Walk Test (10MWT). Walking distance, assessed by 6-Minute Walk Test (6MWT), Physiological Cost Index of walking (PCI) and spatial-temporal gait measurements were used as secondary outcomes. Results: Articles included in this review did not show statistically significant results when using FES for the primary and secondary outcomes. Discussion: The use of FES associated with conventional rehabilitation resulted in changes in speed gait and distance gait, but these changes did not appear to be significant compared to conventional rehabilitation only. Further clinic studies with a larger sample size and a more elaborated protocol would be interesting to be realized.
