Prédire la survenue de l'entorse latérale de cheville à l'aide d'un test fonctionnel ? Validité prédictive du SEBT, HOP TEST et FMS® ?

Abstract : Introduction: Ankle sprains are the cause of 7 to 10% of French hospital emergency room visits and 32 to 74% suffer from chronic symptoms at 7-year intervals. With high economic costs generated by this pathology and the disabling consequences, it is important to set up low-cost screening methods. Objective: Can a functional test predict the occurrence of an ankle sprain? Method: Pubmed, Cochrane, web of science and grey literature databases were consulted. No specific population was targeted, the intervention considered the use of a functional test (SEBT, YBT, hop test, FMS®) to characterize dynamic balance, the comparator was the occurrence of an ankle sprain, the outcome was the decrease in performance on the functional test and their predictive capacity defined by using ROC curves and likelihood ratio. Results: 12 studies were included. Only 6 studies provided the area under the curve of functional tests. The analysis showed that different screening tools can be predictive of ankle sprain: multiple hop test (AUC=0.71; 95%Ci=0.53-0.89), single leg hop test (AUC=0.77; 95%Ci= 0.60-0.95). The use of the SEBT and the YBT seems to depend on the sex of the individuals. The data point to the use of the anterior diagonal (AUC=0.674; 95%Ci= 0.564-0.72) for men and posteromedial for women (AUC=0.78; 95%Ci= 0.61-0.95). Finally, these functional tests seem to better predict non-contact ankle sprain. Conclusion: the data cannot be extrapolated to the whole population and remains sport-specific, anthropometric characteristic of the populations and cut-off defined by the studies. The predictive performance is limited to justify their use alone. The implementation of a predictive model including the use of functional tests, BMI, sports type and the injury mechanism seems more appropriate.
