L'augmentation de la cadence de pas chez le coureur souffrant de syndrome Fémero-patellaire

Abstract : Introduction : Patellofemoral pain is one of the most prevalent (7% to 29%) injury in endurance runners. The increase of the running step rate could be a new way to treat this pathology. This new pattern causes modifications of kinematics and kinetics parameters observed while running, revealing a decrease in the knee joint stress and particularly in the patellofemoral joint stress. The purpose of this review is, through pain, function and return to sport, to determine the clinical effectiveness of this approach in runners suffering of PFP. Methods : The PEDro, PubMed, Cochrane Library and Google Scholar databases were searched until March 2021. Seven articles who were matching with the eligibility criteria of this review were identified and included. Thus, two RCT and five case series were analyzed in order to deal with our research subject. Due to studies and tools heterogeneity, a qualitative synthesis was performed. Results : These data predominantly suggest a beneficial effect of the intervention studied. The RCT show dubious results between intragroup and intergroup evolutions. Conversely, all case series support a positive effect of the intervention on pain while the effect on function is more uncertain. Only two studies are interest in return to sport, giving very few information. Discussion : The insufficient quantity of RCT and the low quality of evidence from the case series don’t allow us to make conclusion about the efficacy of the step rate modification. Thereby, the experimental protocols are very different and hardly comparable. More RCT are needed in order to give more data on this topic.
