L'influence de la pose du pied sur les modifications musculo-squelettiques du triceps sural et sur l'apparition d'inflammations plantaires chez le coureur

Abstract : Introduction: the TACP system from the knee to the foot is a complex anatomical area, linking different structures and particularly prone to injury in running. This system plays a major role in landing and propulsion and one of the main factors that can alter it is the running strike pattern: forefoot (FFS), midfoot (MFS) and rearfoot strike (RFS). Objective: to establish an inventory of scientific knowledge in the literature on the influence of foot placement on musculoskeletal modifications of the triceps surae and on the appearance of plantar inflammations in runners in order to better understand the reasons of injuries of the TACP system and to reduce their incidence. Research methodology: the research was conducted in October 2020 with the following databases: Pubmed, Cochrane, Google Scolar, Researchgate et Sciences Directes; our search equation is: (((foot attack) OR (foot strike pattern) OR (forefoot) OR (midfoot) OR (rearfoot)) AND ((triceps surae) OR (gastrocnemius) OR (soleus) OR (Achilles’s tendinopathy) OR (Achilles’s tendon) OR (tendinitis)) OR (plantar fascia) OR (fasciitis)) AND ((runner) OR (running))). The main eligibility critaria indicate that only studies with at least one TACP system less than ten years old were included. Finally, methodological quality was assessed using the Newcastle Ottawa (for case-control and cross-sectional), Pedro (for RCT) and JBI (for case studies). Results: 10 studies were included to identify both the positive and negative impacts of different running patterns. A very important factor to consider in the development of injuries is the rate of loading to which the joints are subjected, particularly the knee. Among the most common injuries are stress fractures of the tibia, metatarsal, Achilles’s tendinopathy and plantar fasciitis. Discussion/conclusion: the RFS pattern has a higher rate of loading than the others, causing joint damage and stress fractures, with the exception of the metatarsal. An FFS pattern would lead to metatarsal fractures, musculotendinous injuries such as contractures of the sural triceps and Achilles’s tendinopathy. Fasciitis is widely represented in all running patterns. Consider the progression in a change of running pattern as well as the wearing of a suitable shoe.
