La prise en charge de la radiculopathie cervicale unilatérale par le traitement Neurodynamique est-elle plus efficace que la physiothérapie conventionnelle ou l’absence de traitement en termes de douleur et de force de préhension ?

Abstract : Introduction: Unilateral cervical radiculopathy is the consequence of the spinal static’s disorder. Conventional physiotherapy techniques, such as massage or muscle strengthening, focus on the environment of nervous system compared with Neurodynamics which targets the spinal nerve roots. Objective(s): The objective is to determine whether Neurodynamic treatment provides a better effect than conventional physiotherapy and / or no treatment in terms of pain and grip strength. Research methodology: The research was carried out on three databases (Cochrane, Google scholar and ScienceDirect) between 2000 and 2021. The studies had to compare an experimental group (neurodynamic techniques) with a control group (no treatment or conventional physiotherapy). Results and discussion: Four randomized clinical trials were selected with a total of 206 participants. Neurodynamic treatment allows a significant reduction in pain but does not allow a significant increase in grip strength. We observe large confidence intervals concerning both criteria and a strong heterogeneity of the results with regards to the grip strength. Conclusions: A low level of evidence in the results suggests caution in formulating a precise conclusion. More homogeneous studies with a larger sample size and a better quality of evidence seem necessary to confirm the effectiveness of this treatment. The lack of reference to its adverse effects raises questions about the benefit-risk balance for the patients.
