Régulation des dépenses pharmaceutiques et accès au marché des médicaments oncologiques en Europe

Abstract : Pharmaceutical expenditure in oncology represents a significant portion of healthcare expenditure and increases over time. The pressure of spending on our healthcare systems represents a real management challenge for decision-makers and payers who have to use various regulation tools such as innovative contracts. Even if the available literature on this subject is still limited, it seems that performance contracts are more effective than financial contracts. However, several design adaptations could improve them. The stakes are high since the quality and duration of life of patients depend on it. Moreover, the significant medical need in this therapeutic area, makes understanding the factors impacting the access of oncological drugs to the market essential. After studying the factors related to the different dimensions of access according to the Organisation for Economic Co-operation and Development (OECD), we used these same factors to measure the degree of access in Europe and to examine their application in practice. Through the example of nivolumab, we were able to verify that several adjustments are still necessary in order to allow for the measurement of access to medicines to be estimated in a way that best reflects reality.
