Suivi à 8 ans de la prothèse totale de genou Légion® : étude d'une série de 183 cas

Abstract : The Legion® total knee arthroplasty (Smith and Nephew) with an oxinium femoral component was introduced in an attempt to reduce polyethylene and osteolysis normal wear as so to increase the prosthesis lifespan. 183 (180 patients) Légion® postero-stabilized total knee arthroplasty (TKA) were included in this retrospective monocentric study, of an average age of 66.9 years and a minimum following-up period of 8 years. The objective was to assess the survival rates and the various factors impacting them, as well as the clinical and radiological results presented by the TKA. The 8 years survival rate was of 94% in terms of mechanical loosening, and of 89.1% for all causes. A body mass index (BMI) <25 kg/m2, an operating time period ≥90 min and a Hi-fatigue cement were all risk factors for mechanical loosening. At the last checkup, the clinical and radiological results were satisfactory and similar to those of the other studies. The high-flexion group shows better flexion than the standard group (121° vs 116°) at the last checkup. Patellar resurfacing failed to improve the rate of residual anterior pain (p=0.399). The study found only a 4% rate of dissatisfied patients, a lower rate than in the literature. Just over half of the patients (55.6%) considered having a normal knee. With a survival rate of 94% for mechanical loosening at 8 years, this study showed no improvement of survival rates, although there are theoretical advantages. It would be necessary to study patients over a longer period of time to determine whether the Légion® offers greater longevity than the other prostheses.
