Quels sont les facteurs obstétricaux et néonataux retrouvés de façon récurrente en cas de discordance entre le pH au sang du cordon et le score d’Apgar à la naissance chez les nouveau-nés à terme ?

Abstract : During delivery, it is recommanded to sample systematically the cord blood gas and also to asses the Apgar Score at 1 and 5 minutes. However, it has been noted in the literature cases of discordance between these two parameters. The aim of this study is therefore to identify the obstetric and neonatal factors found recurrently in the event of a discrepancy between the pH in cord blood and the Apgar score at birth in term newborns. Objectives: identify the recurring factors associated with neonatal acidosis and the obstetric and neonatal factors causing neonatal acidosis without poor adaptation to life (Apgar Score > 7 at 5 minutes of life). Methods: For this, a study of the bibliography was carried out as well as a study of files in a Marseille University Hospital. Results: The review of the literature showed that the presence of meconium amniotic fluid, a retro-placental hematoma, a birth by cesarean section and pre-eclampsia are at risk of mismatch between the pH in the cord blood and the Apgar score at 5 minutes. The file review was not able to illustrate theses datas. Other parameters such as the morphology of the cord or the methods of gas sampling and measurements can induce a more acidic pH. Conclusion: In conclusion, risk factors for a discordance between cord pH and the Apgar score at 5 minutes of life were put forward, as well as a tendency towards acidosis of the cord sample after birth. Poor quality of gas sampling and evaluation of the Apgar score is one explanation.
