Contribution diagnostique et thérapeutique d’un avis infectieux hebdomadaire dans un service de réanimation pédiatrique

Abstract : Introduction: Primary or secondary infectious diseases are common in Pediatric Intensive Care Units (PICUs). The main objectives of this study were 1/to describe the clinical recommendations given on a weekly basis by an infectious disease specialist in a PICU and 2/to assess its compliance. Methods: An observational prospective single-center study conducted over a 1 year period in a PICU of a tertiary university hospital. All clinical recommendations provided by an infectious disease specialist during a weekly round were collected. Results: We reported a total of 234 clinical recommendations on 148 patients (median age 34 months [IQR 2,5 months – 10,5 years]). Infections occurred in patients with comorbidity in 73 (49%) of them. Most of them had pulmonary infections (n=93, 40%) and patients presented with septic shock in 27 cases (12%). 8% of infections (n=19) were related with multiresistant bacteria. Clinical recommendations were considered as preventive, diagnosis, monitoring and therapeutic in 41 (18%), 98 (42%), 34 (15%) and 205 (88%), respectively. Treatment was optimized in 197 (85%) of cases, including treatment discontinuation (n=63, 27%), spectrum reduction (n=26, 11%) and dosage adjustment (n=15, 6%). The duration of treatment was also optimized in 144 situations (62%). Overall, we found a full compliance in most of cases (n=212, 90%). Conclusion: We found a very good compliance of clinical recommendations provided on a weekly basis by an infectious disease specialist in a PICU. Larger pediatric studies are necessary to assess a potential benefit on patients’ outcome.
