Préservation de la fertilité chez les femmes atteintes de pathologies malignes : accès, facteurs liés à son accès et à sa réalisation

Abstract : Introduction: as part of the law of bioethics and the latest cancer plan, fertility preservation (FP) must be offered to any women whose fertility is likely to be prematurely altered as a result of her oncological treatment. The objective of this study is to assess the proposal rate of FP in women with a theoretical indication for it, as well as the factors related to the proposal, approval and achievement of FP. Methods: this study is retrospective, observational, monocentric. All adult women under 40 who were first treated for a cancer between January 1, 2015 and December 31, 2019 at the Gustave Roussy Institute were included. Results: 792 women were included, and three populations were isolated. All populations combined, the proposal rate of FP is 58.2%. In population 1 (n = 507 ; breast and gynecological cancers, hematologic malignancies) this rate is 83.6%. The other two populations have not clearly recognised indications for FP, their proposal rate is uncommon. The factors limiting the FP proposition are higher age and previous parity, with therapeutic emergency and hematologic malignancies. There is no social barrier to proposal, approval and achievement of FP. Conclusion: in most cases, female cancer patients received a proposal of FP, but this varies according to the indications and types of cancer. The collaboration of multidisciplinary teams is crucial for the care of these women and their follow-up is essential to improve the indications for oncofertility management.
