La formation en gynécologie chez les étudiants sages-femmes d’ïle de France

Abstract : Introduction: Midwives' skills grow over the years, mainly in gynecology. As first specialists of woman physiology, they can support them throughout different life stages, even outside of pregnancy periods. As per the midwives' skill widening and the profession's diversity, we asked ourselves if midwives were sufficiently trained during their studies to handle preventive gynecologic follow-up and contraceptive consultations and the impact of the initial training regarding those practices.
Methodology: We carried out a multicenter quantitative study among midwives who graduated in 2019 from one of the four schools located in the Ile-de-France area (France) via an online survey. We collected 91 answers, which account for a response rate of 50.3%.
Results: We have noticed that 2/3 of midwives felt at ease handling preventive gynecologic and contraceptive follow-ups after their initial training. However, the lack of practice represents a drag on midwives' satisfaction during their studies and gynecology initial training. Most respondents highlighted that practice shortfall. At the end of their training, 86.8% of midwives in our study chose the hospital sector, against 82.4% a year after. The vast majority is working in the delivery room, at a rate of 79%, and in postpartum, at 55%. Despite that, gynaecologic consultations remain less accessible to midwives working at the hospital. The main observed activity is obstetric.
Conclusion: Finally, global gynecologic knowledge learned during the initial training's theoretical courses is recognized mostly favorably, even though some gaps and deficiencies of more in-depth development are also mentioned. The lack of practice during initial training remains the most critical outcome of our study, as well as limited access to gynecologic practice in the hospital sector, where most young graduates start to practice.
