Résumé

INTRODUCTION: Decisions regarding the withdrawal of life support are an integral part of the daily life in hospital. The role and involvement of the GP in making these decisions is neither clearly defined by law nor in the medical guidelines. METHODS: Qualitative study during a semi-structured and individual clinical consultation of GP using an interpretative phenomenological analytical approach. The main assumption was to identify the involvement of the GP in the decision of discontinuing the treatment. The second assumption was to determine whether it could be an asset for the GP to have their patients’ advance directives regarding end-of-life issues. RESULTS: Ten clinical consultations were carried out. Two main topics were brought up: the link between the GP and the hospital, and the capacity linked to GPs’ skills. Only one participant had been invited to attend a symposium on the living will. Advance directives are rarely discussed during appointment with the GP. CONCLUSION: On the one hand Hospitals fail in involving GPs in withdrawal of life support and on the other hand the GPs fail in managing the issue. Improvements could be made: firstly, by developing better communication between the hospital and the GPs. Secondly, by creating awareness about advance directives among the general public and with the GP including an increase in salary. Thirdly, by offering training courses in palliative care.
INTRODUCTION : Les limitations et arrêt de traitements (LAT) font partie intégrante du quotidien médical hospitalier. L’implication et la place du médecin traitant dans ces décisions n’y sont pas définies par le cadre légal et les recommandations. MÉTHODE : Étude qualitative en entretien individuel semi-dirigé de médecins généralistes installés. Analyse interprétative phénoménologique des verbatims. Hypothèse principale : Déterminer l’implication des médecins traitants dans les décisions de LAT dans le cadre hospitalier. Hypothèse secondaire : La possession des directives anticipées par le médecin traitant est un atout pour valoriser son implication dans les LAT. RÉSULTATS : Dix entretiens ont été réalisés. Deux méta-thèmes ont été identifiés : Lien ville-hôpital et aptitudes liées à la pratique de la médecine générale. Un seul participant a assisté à une LAT. Les directives anticipées ne sont que très peu abordées en consultation de médecine générale. CONCLUSION : Manque d’implication aux décisions éthiques du médecin traitant par les praticiens hospitaliers mais également liée à la pratique des médecins généralistes. Plusieurs axes d’amélioration possibles : premièrement, développer la communication ville-hôpital. Deuxièmement, sensibiliser la population générale ainsi que les médecin traitant aux directives anticipées. Et troisièmement, revalorisation de la pratique et formation des médecins généralistes en soins palliatifs.
