Étude comparative de différentes méthodes d’évaluation de la sensibilité aux antibiotiques chez les bactéries du groupe Bacteroides fragilis ayant acquis un ou plusieurs mécanismes de résistance

Abstract : Bacteroides from the fragilis group are the most frequent anaerobic bacteria isolated in bacteriemia and deep infections in Humans. These infections are treated by antibiotic agents such as metronidazole, penicilins with penicillinase inhibitors, carbapenems or macrolides. Despite adequate treatment, these infections are the cause of high mortality rates. Moreover, since the early 2000s, resistance within Bacteroides has been increasing, leading to more frequent treatment failures. The aim of this work was to compare different antibiotic susceptibility testing methods, used routinely in laboratories, with the reference method, the agar dilution method. The evaluated methods were two industrial kits of microplate broth microdilution method (Sensititre™ and Micronaut-S™) and the agar disk diffusion method. The strains tested came from a European collection of 55 Bacteroides fragilis group presenting one or more resistance mechanisms. Eleven frequent resistance genes have been searched in the collection and related to the phenotypic resistance. At the end of this work, results suggest that the disk diffusion method was the most efficient to determine the Bacteroides resistance profile, in particular to macrolides and penicillins with inhibitor, if a cut-off diameters increase are made for the last one. Meropenem has also been found to be more efficient than imipenem to determine carbapenems resistance.
