Modifications techniques du lambeau bipédiculé palmaire en îlot de Tranquilli-Leali et son apport dans la prise en charge des reconstructions pulpaires pour les amputations distales des doigts longs

Abstract : Introduction: the use of flap in fingertip reconstruction with bone exposure is still widely accepted. The authors present a modification of the Tranquilli-Leali flap. The objective of this study is to assess the sensory, aesthetic and functional results. Patients and methods: we retrospectively analyse the prospective data of 42 patients with a mean follow up of 16,4 months (30-6 months) operated in two centres. 33 were men. Mean age was 39.5 years (12-83 years old) and 57.1 % (n=24) were smokers. In 54.8 % (n=23), the traumatism was work related. Crushing mechanism was involved in 69 % (n=29). According Allen’s classification, we report 26 cases of stage II, 14 cases of stage III and 2 cases of stage IV. Results: no claw nail deformity was observed. The mean Weber was 3.8 mm (2-6 mm). The mean TAM was 268.3° (255-275°). We report 8.7 satisfaction rates on a 0 to 10 scale. We report no vascular complication such as dorsal skin necrosis. Conclusion: our modified Tranquilli-Leali flap allows simple and safe use of the old Tranquilli-Leali flap which keep a bad reputation in the hand surgeon community this flap insures good results providing a balance between skin, bone and nail in Allen stage II and III.
