Représentations et attitudes des patients vis-à-vis de la prescription de paracétamol en soins primaires

Abstract : Objective: In France, paracetamol is the best-selling drug but also the most prescribed: 84% of boxes are delivered on prescription. We therefore decided to study the prescription circuit for paracetamol, little explored so far. The objective was to clarify the representations and attitudes of patients towards the prescription of paracetamol in primary care. Method: We carried out a qualitative study, based on 15 semi-structured interviews, with patients in Ile-de-France. Recruitment took place by snowball effect and in the waiting room of a Health Care Access Center. Results: Different prescribing frameworks emerged from our work: prescribing in an acute context, automatic physician's prescription, placebo prescription and prescription at the patient's request. Satisfaction with this prescription was linked to patients' confidence in their general practitioner. It was also linked to the physician's attitude and his explanations. The patients mainly used the prescribed paracetamol for self-medication. Overall, they had a positive view of paracetamol, qualifying it as the most common drug, harmless, with disputed effectiveness. The notion of symptomatic treatment has been criticized by some patients, considering that hiding their problem could be harmful. The demand for etiological treatment and accurate diagnosis from the physician was strong. Conclusion: We were able to demonstrate patient satisfaction with the prescription of paracetamol, mainly linked to a good relationship with their general practitioner.
