Paramètres d’imagerie TDM quantitatifs innovants dans le diagnostic et l’évaluation du syndrome de bronchiolite oblitérante post-greffe de cellules souches hématopoïétiques

Abstract : Background: quantification of air trapping by parametric response mapping (PRM), an innovative method based on voxel analysis on a biphasic chest CT, has been shown to be of interest in aiding the diagnosis of bronchiolitis obliterans syndrome (BOS) after hematopoietic stem cell transplantation (HSCT). Other quantitative CT parameters still under investigation could be useful, such as lung density variation during the respiratory cycle (DeltaHU), percentage of voxels with density variation less than 100UH (DeltaHU<100), Jacobian determinant (DetJ), anisotropic lung deformation index (ADI) and quantification of expiratory air trapping by spectral clustering (%LDAA). Purpose: to investigate the role of PRM, DeltaHU, DeltaHU<100, DetJ, ADI, and %LDAA in the diagnosis and evaluation of BOS after HSCT. Materials and methods: 27 BOS and 39 non-BOS patients were included. All had undergone HSCT in the setting of hematologic disease, and had biphasic chest CT and pulmonary function tests (PFTs). Imaging parameters were studied on the total lung field and then by region. Finally, a correlation study with the data of the EFR in BOS patients was performed. Results: on the total lung field, DeltaHU and ADI were significantly lower in BOS patients while %LDAA was higher (p=0.01). In the lower regions, DeltaHU and ADI were lower in BOS patients (p<0.01) while DeltaHU<100 was higher (p<0.01). Significant correlations were found between quantitative CT variables and EFR parameters in BOS patients. Conclusion: deltaHU, DeltaHU<100, ADI and %LDAA may be useful in the diagnosis of BOS after
