Épilepsie associée aux gliomes cérébraux : revue générale et identification expérimentale de mécanismes de déclenchement des crises

Abstract : Brain tumors, and more particularly diffuse gliomas, are highly epileptogenic. Epidemiologic and experimental studies point that tumor growth and epileptogenesis are associated, sometimes in a contradictory manner: tumors with a specific growth pattern are more epileptogenic; epilepsy occurrence is associated with an improved oncologic outcome increased neuronal activity promotes tumoral growth; common mechanisms mainly involving glutamatergic excitatory signalling and GABAergic inhibitory signalling contribute to both epileptic and oncologic process. This study aims at better understanding two major protagonists involved in seizures initiation: extracellular K+ and neuronal Cl- concentrations dynamics that respectively underlie local excitability and inhibitory versus excitatory GABAergic signalling polarity. These experiments were conducted ex vivo in peritumoral epileptic human cortex. They focused on the KCC2 and NKCC1 Cl-/K+ cotransporters that regulate Cl- and K+ concentrations. Their expression is altered in epileptic tissue leading to excitatory GABA effect. I have tested the impact of their modulation in dynamical conditions at seizures emergence. The results show that dynamical and complex interactions between extracellular K+ and intracellular Cl- concentrations is a key phenomenon of seizures occurrence. This phenomenon is increased by basal changes in epileptic tissue, which opens new therapeutic perspectives.
