Prise en charge de l’agitation en réanimation : revue de la littérature et présentation du protocole SARA - suivi de l’agitation en réanimation après mise en place d’un algorithme

Abstract : Agitation is a common phenomenon in intensive care unit (ICU). Contributing factors can coexist: pain, underlying disease, withdrawal, delirium and some medications. Agitation can compromise patient safety (self extubation, removal of catheters...) and increase ICU length of stay. Little is known regarding its management and the clinical practice guidelines for the management of pain, agitation, and delirium in adult patients in the ICU do not describe the choice of drugs. There is also no standardized protocol for the management of agitation in the Centre Hospitalier Universitaire de Grenoble-Alpes (CHUGA). The aim of this work is to improve the management of the agitation. A literature review leaded us to suggest recommendations for safe and effective treatment. Only few drugs can be used for the elderly patients or patients with cerebral damage. We then designed a before-after study to test a clinical tailored ICU agitation protocol,, in the polyvalent surgical ICU of the CHUGA. Epidemiological data and the impact of the protocol on the duration of agitation will be described.
