Réflexions sur l’organisation du travail des médecins généralistes à travers leur nombre de patients et la qualité des soins

Abstract :
Introduction. In France, the medical demography is decreasing while the population is increasing. General Practitioners are overwhelmed with an average consultation time of 18 minutes only. Increasing the number of daily consults could lead to a deterioration in the quality of care, discontent among doctors, increase of medical malpractices, etc. The objective of this study is to determine what link could exist between the quality care and the number of patients seen daily by a general practitioner in a metropolitan area.
Method. A unique questionnaire was sent to General Practitioners, between December 2017 and June 2018. The main judgment criterion was the satisfaction of the general practitioners, in a metropolitan area.
Results. 86 General Practitioners answered. The men among them declared seeing an average of 29 patients a day when women declared seeing 24. A correlation has been found between the number of daily consults and the environment, the deterioration in the quality of life at work, satisfaction with compensation and the time given for explanations or taken to reassess difficult situations. No correlation were found with the satisfaction of general practitioners.
Discussion. There is no unique link in between quality of care and number of daily consults. However, this study allows us to perhaps re-think the work organization: a unique and bilateral method doesn’ t exist, but has to be specific to each General practitioner and patient. The arrival of the new generation of general practitioners could be supervised and they could be better supported in the career project.
