Traitement de la tumeur primitive dans les carcinomes épidermoïdes des voies aérodigestives supérieures métastatiques d’emblée

Abstract : Patients with synchronous metastatic head and neck squamous cell carcinomas (HNSCC) are at risk of developing locoregional progression associated with significant morbidity and mortality. The aim of this study is to assess the role of locoregional treatment directed to primary tumour when associated with systemic therapy as first-line treatment.
This retrospective study included 148 patients from Gustave Roussy and presenting with synchronous metastatic HNSCC. 80 patients were treated with systemic therapy alone and 68 patients were treated with a combination of locoregional treatment and systemic therapy. Median overall survival (OS) was 13 months [1-123] and median progression free survival (PFS) was 10.1 months [1-67]. Median OS was significantly improved in patients treated with locoregional treatment compared to patients treated with systemic therapy alone (median OS 16.8 [IC95%:13-22] vs. 7.5 months [IC95%:5.6-13.1] respectively, p<0.001). Median PFS was also significantly improved (median PFS 14 [IC95%: 10-22.5] vs. 8.7 months [IC95%:6.5-10.1] respectively, p=0.001).
Among patients displaying partial or complete response to initial systemic therapy, locoregional treatment was associated with prolonged OS and PFS (median OS 24.5 [IC95%:14.8-32.1] vs. 15.8 months [IC95%:9.7-17.5], p=0.045 and median PFS 18.6 [IC95%:10.9-26.9] vs. 9.2 months [IC95%:6.6-19.1], p=0.05). The addition of a locoregional treatment to systemic therapy is associated with prolonged OS in patients with synchronous metastatic HNSCC.
