Recettes et stratégies des médecins deprescripteurs

Abstract : Polypharmacy in our society leads to numerous adverse effects and drugs interactions, inducing morbidity and mortality that should not be neglected. Deprescription can reduce drug-related side effects to improve patients’ health. However, this procedure is difficult to follow for general practitioners, who are experimenting several seemingly insurmountable obstacles. The aim of this study is to explore deprescribers’ strategies to help their peers in deprescribing.
This qualitative research is based on individual semi-structured interviews of general practitioners who deprescribe in Ile-de-France. The analysis of the written transcriptions by grounded theory led to modelling of a coding tree.
Deprescription is set for these doctors in a global reflexive procedure, that makes their daily practice rewarding. Continuous training gives them the opportunity to question their knowledge. A trust-based relationship with their patients is necessary to start the deprescription, through motivational interviewing and therapeutic education. Patient and doctor need to find the appropriate time for both, while ensuring the continuity of care. Each consultation can be the occasion to review pathologies so as to re-evaluate the treatments. Communication with other healthcare professionals can improve comprehensive care of the patient.
Through this study, practitioners share their energy and pleasure to practice deprescription. The guide coming from this research can sensitize general practitioners to deprescription. Everyone can become a deprescriber.
