Efficacité des patchs de capsaïcine dans le traitement des douleurs chroniques du patient âgé de plus de 70 ans

Abstract : Introduction: chronic Pain in the elderly is a cause of reduced autonomy that can lead to a state of functional dependence. Capsaicin patches, usually used for peripheral neuropathic pain in adults, reduce the iatrogenic risks associated with age and the systemic passage of most analgesic therapies. Objectives: to study the effectiveness of capsaicin patches for all types of chronic pain in patients over 70 years old. Materials and methods: in this retrospective single-center study carried out at the Bordeaux University Hospital, 58 patients were included between January 1, 2010 and December 31, 2020. The data were collected from computer files. The primary endpoint was reduction in chronic pain by more than 2 points on the numerical scale. The secondary endpoints correspond to the factors that may be associated with altered efficacy of capsaicin patches. Results: The efficacy of the capsaicin patches was 50% for all causes. If the treatment was effective for the etiologies for which it is indicated, the results are also convincing for pain of osteoarthritis (46.2%), post-surgical (40%) and due to nervous conflicts (40%). Finally, a statistically significant association of increased efficacy is found for obese patients (BMI> 30). Conclusion: the use of capsaicin patches in chronic pain from all causes in people over the age of 70 has shown promising results which pave the way for an extension of the indication.
