Malformations artérioveineuses des maxillaires : présentation d'un cas clinique

Abstract : Intraosseous arteriovenous malformations are rare congenital vascular structure defects, especially if they involves the maxillary. Their low incidence makes it a little-known pathology. The diagnosis is therefore sometimes complex and slow, even though AVM usually manifests itself in hemorrhage, which is difficult to control. A set of clinical and paraclinical examinations makes it possible to highlight the AVM nidus supplied by afferent arteries and drained by arterialized efferent veins. Once the angioarchitecture of the AVM has been established, management is mainly done by embolization and / or surgical excision. The excision is of the carcinological type, eventually associating bone and dental defects requiring complex rehabilitation. These bone defects require bone grafting usually performed using autologous samples extraoral. It seemed important to describe these types of bone rehabilitation that allow the creation of implants for a fixed dental rehabilitation. Removable maxillofacial prostheses also rehabilitate the orofacial mass. In order to highlight the advantages and disadvantages of an autologous transplant with extra-oral sample, we have retraced the care path of a patient treated at the Timone U.H.C in Marseille. The main difficulties concerned the embolization product : radiographic artefacts, restricted vascular network and interactions with nonabsorbable material.
