Vécus et représentations des médecins généralistes dans la prévention et la prise en charge des déformations crâniennes positionnelles du nourrisson : étude qualitative menée en Seine-et-Marne

Abstract : Background: Since the recommendations on the prevention of the sudden infant death syndrome, the increasing prevalence of the Positional Cranial Deformities (PCD) generates parental anxiety, questioning the back lying and the lack of prevention of PCD by general practitioners. The objective is to explore the representations of general practitioners on PCD, through their experiences, in order to identify possible obstacles to the optimization of prevention and management of PCD. Methods: A qualitative study was carried out with 13 general practitioners in Seine-et-Marne. Results: The representations of the participating practitioners were as heterogeneous as their practices. The management depended on their representations of PCD during their experiences. Discussion: Several obstacles restrain general practitioners from improving the prevention of PCD: the lack of knowledge, the lack of classification of the severity’s level of PCD, heterogeneous representations of PCD, the relationship between the parents and their doctor, misinformation by the media and the lack of collaboration between health professionals. Beyond these obstacles, the attitude and practices of general practitioners seem to depend on the awareness of the general practitioners during their experiences. Conclusion: Awareness campaigns of the HAS 2020 recommendations during the initial and ongoing training of doctors would be decisive to encourage general practitioners to the primary and secondary prevention of PCDs, while respecting the sudden infant death syndrome’s recommendations.
