Évaluation de la nécessité d’un drainage en chirurgie carotidienne : résultats préliminaires de l’étude CASANDRA

Abstract : Objective: Evaluate the absence of risk of occurrence of cervical hematoma requiring surgical revision in the absence of drainage during carotid surgery. Method: 121 patients were included over a period of 16 months at the Caen University Hospital, including all the patients who had not been drained during a carotid thromboendarterectomy. The main outcome measure is surgical revision for a cervical hematoma. The secondary objectives were to assess the factors favoring the appearance of these hematomas, the length of hospitalization, the late complications at 3 months. Results: Only 2 patients out of the 121 (1.6%) presented with a cervical hematoma that was surgically revised. Morphological parameters, medical history or antiaggregants or antihypertensive treatments did not affect the revision surgery for cervical hematoma. Only the value of the PT, below 0.82, appears to be an increased risk of postoperative cervical hematoma (p<0.03). The pre, per or post operative data have no effect on the occurrence of post operative hematoma. The TCMM is evaluated at 2.4% including 3 deaths including a hemorrhagic stroke. The length of hospital stay was 1.92 days and 1.31 days after adjustment. Conclusion: The lack of drain after carotid surgery is a safe technique. There is no increased risk of developing postoperative cervical hematoma. Only the PT value of less than 0.8 appears to be an increased risk of developing postoperative cervical hematoma. The TCMM of the CHU de Caen is lower than the recommendations of European companies. The length of hospital stay in the absence of drainage is short.
