Comment les médecins généralistes font-ils pour repérer les troubles du spectre de l'alcoolisation foetale à La Réunion ?

Abstract : Background: Fetal Alcohol Spectrum Disorder (FASD) is the leading cause of non-genetic mental disability and social maladjustment in children. It is entirely preventable. The national recommendations recommend zero alcohol during pregnancy. Reunion Island is the most affected French department with an estimated FAS prevalence of 1.2‰ births. The prevalence of FASD is not known. Its prognosis is greatly improved by early medico-psychosocial management. The objective of this study was to understand how general practitioners (GPs) did to identify FASD in Reunion Island. Method: Qualitative study using a grounded theory approach, through semi-structured interviews with general practitioners. Results: Twenty interviews were conducted. The GPs reported obstacles to the identification of FASD: lack of knowledge and experience, lengthy consultations, non-specific and highly variable symptoms, and a lack of consensus concerning the classification and method of diagnosis involving the mobilization of a multidisciplinary team. They did not think to identify FASD in adults and doubted the usefulness of treatment, as this disorder is irreversible. Conversely, they felt competent to identify neurodevelopmental disorders of all causes but were concerned about the long delay in specialized care. They noted a more pronounced denial of alcohol use among women. The identification of their alcohol consumption was not always carried out systematically during pregnancy. Conclusion: promotion and training in the identification and coordination of care for children with neurodevelopmental disorders of all causes should be given priority.
