Extrême prématurité : état des lieux, prise en charge et morbi-mortalité en Normandie occidentale

Abstract : Introduction: extremely premature infants (born before 26 weeks) survival rates are lower in France. Our aim was to depict a state of play of extremely premature births in the Manche-Calvados-Orne Perinatal Network while focusing on their management, mortality and morbidity. Materials and methods: we conducted a retrospective study of live births through 230/6 weeks to 256/6 weeks between 01/01/2015 and 12/31/2019 in the eleven maternity wards of the western Normandy Perinatal Network, without home births. Results: 90 women and 103 infants were included. 84 infants received active care and 42 survived. Active management was associated with gestational age (only 4 infants born at 23 weeks received active care), prenatal corticoids, fetal heart rate monitoring and c-section. Prenatal corticoids and fetal heart rate monitoring were also associated with gestational age. Active neonatal and antenatal care only depended on gestational age, with a threshold of 24 weeks, and was not associated with other prognostic factors. Mortality in neonatology ward was not associated with gestational age but with in utero exposure to magnesium sulfate and antibiotics and with postnatal corticoids. 35 infants out 42 survivors had a severe morbidity. Conclusion: active management of extremely premature infants depended on gestational age in our region, with a threshold at 24 weeks.
