Thèse qualitative sur le vécu des médecins généralistes du Calvados face au mésusage d'antalgiques opioïdes

Abstract : Introduction: The use of opioid analgesics contributes to improve pain management. However, these drugs are at risk of misuse, in particular because of their addictive potential. The increasing rate of intoxications in these recent years in France exhorts caution. General practitioners being the first prescribers of opioid analgesics, we were interested in their experiences as well as their practices of opioid analgesics misuse. Method: We carried out a qualitative study by semi-structured interviews with 12 general practitioners located in Calvados. Opioid analgesics reimbursement data from 2016 to 2020 were also retrieved. Results: Practitioners encounter difficulties due to the lack of training, time, and access to specialized consultation in addictology or pain assessment. Multidisciplinarity and the role of the general practitioner in the care process are identified as assets. The practices of physician in Calvados are in accordance with the good practice recommandations concerning knowledge of misuse risk factors and warning signs during prescriping. However, screening for risk factors is not systematic and information on the risk of dependence is rarely given to the patient. Conclusion: In order to limit situations of misuse, practitioners ask for better training in addiction and pain and improved coordination between general practitioners and the addiction care network.
