Choix des biomatériaux dans la fermeture secondaire des fentes alvéolo-palatines chez l'enfant : revue de la littérature

Abstract : Introduction: The aim of this study was to identify the alternatives to autologous bone graft in secondary alveolar bone grafting for children with alveolar cleft palate. Materials and methods: An electronic search was conducted via PubMed/MEDLINE and Cochrane Central. Results: 19 articles were include, 476 patients and 497 alveolar bone grafts performed. The average age at the time of the bone grafting was 8.8 years. The bone defects were small (0-1cm3) to moderate (1-2cm3). In 48% of cases an autogenous graft was performed, in 31% a bone substitute alone and in 21% an autogenous graft was combined with a bone substitute. For βTCP, rhBMP2 and xenograft type bone substitutes alone or in combination with an autogenous graft, bone continuity was achieved in over 80% of patients. For bioceramics in 63.8% of patients and for allografts 57% of patients Conclusion: Autogenous iliac grafts remain the gold standard for secondary closure of alveolar clefts. The use of a bone substitute such as xenograft, bTCP or rhBPM alone or combined with an autogenous graft can be a therapeutic alternative to autogenous graft alone in secondary closure of alveolar-palatal clefts. Allografts should be used in combination with an autogenous graft. Bioceramics and allografts alone should be used with caution given their poor performance compared to other biomaterials.
