Efficacité cérébrale de l'association lomustine, cyclophosphamide et étoposide dans le traitement de deuxième ligne et plus des carcinomes neuroendocrines pulmonaires métastatiques

Abstract : Lung cancers are the third most common cancer in France and the leading cause of cancer deaths. Neuroendocrine lung cancers have a poor prognosis with a 5-year survival rate of less than 10% for all stages combined. First line of treatment for these cancers is well defined with the combination of platinum and étoposide with radiotherapy in case of localized disease or with atezolizumab in case of metastatic disease. In contrast, there is no gold standard for patients in second-line or more metastatic setting. The choice is therefore made on the basis of clinical characteristics such as the presence of brain metastases, which are present in up to 30% of cases of this cancer type. The main objective of this work is to evaluate the cerebral efficacy of the combination of lomustine, cyclophosphamide and étoposide in the treatment of these patients in second line or more. 66 patients were retrospectively evaluated. The intracerebral response rate was 56.3% and the intracerebral disease control rate was 81.7% according to RANO criterias with a mean intracerebral response time of 3.28 months. The overall survival of the patients in the cohort was 6.1 months. The safety of this treatment was acceptable with only 10 CTCAE grade 3 or 4 toxicity events and only 2 treatment discontinuations for toxicity. Lomustine, cyclophosphamide, and étoposide treatment appears to be the treatment of choice for second-line or higher treatment of brain metastatic pulmonary neuroendocrine carcinoma, given its efficacy and safety.
