Mesure cervicale échographique et risque d’accouchement prématuré chez les grossesses gémellaires asymptomatiques

Abstract : Objective: The purpose of our study is to measure the interval between a short cervical length below 25 mm, antenatal corticosteroids and the delivery for twin asymptomatic pregnancies. Those data can show if the exposure of antenatal corticosteroids is optimal with a delivery within 7 days of the cure. Methods: A descriptive multicentric and retrospective study was carried out from 2007 and 2017 for asymptomatic twin pregnancies with one or more short ultrasonography cervical length below 25 mm and before 34 weeks, whether there is a corticosteroids cure or not. Results and conclusion: This study allows us to include 137 asymptomatic pregnancies with short ultrasonography cervical length below 25 mm and before 34 weeks+0 day. Our data showed that we had a mean and median interval of between 44 days and 37 days, higher than 7 days between the measure of short cervical length and the delivery. Only 4 patients of 137 had a delivery within the 7 days that represent 2,9%. Then, 56 patients of 137 were exposed to antenatal corticosteroids which is 40,8%. We observed a mean and median interval of 43 days and 41 days between the corticosteroids and the delivery. To conclude a cervical length under 25 mm without symptoms, doesn’t seem to be predictive of a delivery within the 7 days. An important part of asymptomatic twin pregnancies seems to have a suboptimal exposure of antenatal corticosteroids, but other studies are needed.
