Mise en place d’une consultation post-réanimation multidisciplinaire dans une unité de réanimation thoracique : évaluation du syndrome post-réanimation

Abstract : Objectives: although mortality of intensive care unit (ICU) patients has declined significantly in recent decades, morbidity after admission remains a significant issue. Post‐intensive care syndrome (PICS) refers to physical, cognition, and mental impairments that occur during ICU stay, after ICU discharge or hospital discharge, as well as the long‐term prognosis of ICU patients and their families. The post intensive care consultation (PICC) is one of the ways to diagnose and treat the PICS. Our objective was to estimate the incidence of physical and psychological issues diagnosed at the PICC 3 to 6 months after surgical intensive care unit discharge. Methods: this observational monocentric retrospective study was conducted between December 2018 and March 2021. We included all major patients with a length of stay > 2 days. The PICC is a multidisciplinary consultation with an intensive care doctor, a psychologist and a physiotherapist. The Short Form 36 (SF-36), the Hospital Anxiety and Depression Scale (HAD) and the Index of Event Scale Revisited (IESR) are completed. The memory tool is used to explore the patient’s memories. Results: The data of 55 patients were analyzed. 89% of them presented a PICS. 65% had a psychological issue and 85% a physical issue. Conclusion: we found a high incidence of PICS in our population. The PICC is a way to diagnose and treat the complications of ICU. But it is difficult to extend to all ICUs because of its human and financial costs. Hence the importance of an early prevention of the PICS through psychological support in intensive care and the ICU diaries. At the discharge of ICU the link with the general practician (GP) is essential to assure a good follow up to the patient. We have to work on this link and make of the PICC a place of exchange between ICU and GPs.
