Utilisation des antibiotiques en période anténatale lors des ruptures prématurées des membranes à terme : étude comparative dans deux maternités parisiennes

Abstract : Objective: To compare two attitudes of service in the antenatal administration of antibiotics during premature rupture of membranes at term. Materials and Methods: Retrospective study, two-center conducted in 2007 in two maternity hospitals in Paris: Port-Royal (Paris 14th), Louis Mourier (Colombes 92), which included 183 mother-infant pairs in Port-Royal, 295 in Louis Mourier. Patients are all premature rupture of membranes at term older than 12 hours. Those of Port-Royal receiving antibiotics systematically from 12 hours of rupture. At Louis Mourier, antibiotics are administered selectively only when signs suggestive of chorioamnionitis. The outcomes maternal and fetal infection were analyzed and compared between the two pregnancies. The group B stretococcus screening has not been studied. Results: The antibiotic was administered at 79.24% of patients during prenatal against 4.41% in the targeted delivery. The antibiotic appears to play a protective role in reducing the risk of infection in utero induced by the length of rupture of membranes prolongated. However, we found rates of neonatal infections identical in both maternity 4.92% and 4.41%, and the difference in the incidence of infections associated with group B streptococcus was not significant. The rate of Gram-negative bacteria resistant to ampicillin was 5.13% with the routine administration of antibiotics. Conclusion: In the absence of new evidence sufficient to guide one voice professional practice, a limited use of antibiotics seems to be a reasonable approach, given the risk of resistance of antibiotic prophylaxis in cases of premature rupture of membranes to terms
