Élaboration d'un protocole d'étude évaluant l'incidence et la prévention de la maladie thromboembolique veineuse en stations de sports d'hiver‎

Abstract : The question of VTE prevention in lower limb injuries arises every day for the moutain physicians. Indeed, 90% of the wounded are treated as outpatients and general practitioners are left alone to assess the need to prescribe LMWH as there is no formal recommendation in ambulatory traumatology. In 2004, the MdM association created an algorithm to frame LMVH prescription, based on the few studies that have demonstrated a significant reduction of DVT with LMWH for patients with lower limb trauma. To assess these practices, it was important, firstly, to know the incidence of VTE in this particular population, mostly tourists, and secondly, to evaluate this algorithm. We developed a study protocol to assess the incidence and prevention of VTE. A feasibility study was more than necessary to develop such a protocol. On one hand, we conducted a study on thirty wounded wich allowed us to refine the questionnaires for a better understanding and to improve the study effectiveness. On the other hand, a study on doctors has forced us to revise the survey design to maximize physicians' compliance. Indeed, one of the major hurdles of the study was to overcome the issue regarding the obvious lack of time during winter season, therefore it was necessary to streamline the initial protocol of inclusion. We have also developed means of communication to inform all physicians and enable their recruitment. Thus, the study "M2M-thrombosis" was able to be created and will hopefully clarify recommendations to prevent VTE in ambulatory traumatology.
