Étude d'impact d'une action de formation médicale continue sur la prise en charge du patient insomniaque en médecine générale

Abstract : Sleeping disorders are a major problem of public health. The number of general practitioners decreases. The improvement of knowledge and practices in family practice is important. This study estimates the efficiency of an interactive training program, organized by the "Collège National des Généralistes Enseignants" : "How to improve the management of the patient who sleeps badly? ," in France, in 2007-2008. In this declarative observational retrospective study, the practice of 152 trained doctors and 152 control doctors were compared, on 10 items from the recommendations of the Haute Autorité de Santé (HAS). The results are shorted in a score on 100. In a statistically significant way, trained doctors are more in adequacy with the repository of the HAS (score : 59,2 +/- 15,3 versus 52,1 +/- 14,7). This significativity persists after the adjustment of a variable of confusion identified (status of student mentor). In case of chronic insomnia, trained doctors assessed they used a sleep diary, natural methods and psychotherapic means, more frequently. This study shows that training with this program is associated with a declared behavior more complying with the repository. These results would need an advanced "before / after" study in order to assert a causal link between training and behavior changes. It remains to prove if these statements are a reality in daily practice and if they influence the patient's health.
