Évaluation de la prise en charge des coliques néphrétiques aux urgences de Grenoble sur l'année 2010, par rapport à la conférence de consensus de 2008

Abstract : Primary Objective: evaluate professional practices in the management of renal colic in the Grenoble E.R. in 2010, compared to the recommendations issued by the updating 2008 of the conference consensus of the French Society of medical emergencies. Secondary objective: describe diagnostic management in patients over 60 years old with acute low-back/abdominal pain syndrome, for which diagnostic uncertainty is important. Method: Retrospective study over 2010, involving 481 patients having being discharged with a diagnosis of renal colic. Results: In acute, only 66% of patients are prescribed NSAIDs, and only 28% morphine. However, other non-recommended treatments are widely prescribed as antispasmodics in 52% of cases. Outpatient, NSAIDs are widely prescribed (80%), but the treatment time is too short. 16% of patients do not receive any output treatment. When dealing with complicated renal colics or patients over 60 years old, imaging is more frequently performed in emergency conditions. For simple renal colics, the combination of Abdominal X-Ray and ultrasound is widely prescribed (40%). At the exit of the E.R., supervisory boards are missing. Conclusion: Although management of renal colics is very clearly and thoroughly regulated, the procedure is not always duly respected. The acute treatment increases the use of molecules, which have not yet been proven to be effective. Imaging is faltering and does not follow the pattern prescribed in the case of complicated renal colics and in patients over 60 years old.
