Influence du stage ambulatoire de niveau un sur le projet d'installation des internes en D.E.S. de médecine générale de la faculté de Grenoble

Abstract : Background: The practice of general medicine is meeting a major demographic crisis. Today, for the younger generations of general practitioners, working in a private practice isn't as tempting as it was before. Objectives: To evaluate the influence of the first ambulatory training course on the postgraduate medical students' projects of creating a private practice of general medicine in the medical university of Grenoble (France) and on the characteristics of this project. Methods: We conducted a monocentric prospective, declarative and comparative study. We included and questioned before and after the training course the 47 interns studying general medicine in Grenoble who were finishing their first ambulatory training course between May and October 2011. Surveys concerned the students' professional projects and the progress of the medical ambulatory training. Main results: 25 students (53,2%) answered both surveys. The first ambulatory training course consolidate interns in working at least partially in a private practice with 28% of the interns wishing to work in a private practice and 60% wishing to work both in a private practice and as an employee in a health care structure at the end of the training course. Even if there is no significant increasing in projects of creating a private practice of general medicine (p=0,46). On an individual point of view 96% of the interns changed at least one of the characteristics of their professional project by the end of this training.80% of the interns expressed their satisfaction about this training and many underlined the positive role of their mentors. Conclusion: This first ambulatory training course is essential in the creation and evolution of the professional projects amongst interns studying general medicine. It allows the interns to adjust their expectations to the reality of a private practice.
