Adaptation des traitements chez la personne âgée hospitalisée‎ : la justification dans le courrier de sortie est-elle vraiment un atout ?

Abstract : In the elderly, to prioritize medical problems and therapeutic suggestions is a public health major issue to prevent iatrogenic disorders and care induced pathologies. A hospitalization in a geriatric department is a privileged moment to adapt the prescriptions. Those therapeutic modifications seem not to be always followed by the general practitioner at home. During one month, we have comparatively analyzed the prescriptions on admission and at discharge, as long as the information contained in the discharge summary, of all patients older than 65 hospitalized in the geriatric department of Voiron's hospital. A one month monitoring was subsequently made to collect the new prescription of each patient. The transferred patients (other acute care departments, surgery, rehabilitation unit) or dead ones were excluded. Thirty seven patients aged of 84.6 ± 6.1 years old were included. There were on average 8.3 ± 3.9 drugs on the prescription on admission and 8.9 ± 3.8 drugs at discharge (p=0.47). 132 drugs were modified. A justification was found in the discharge summary for 67% of interrupted drugs and 80% of inserted drugs. We cannot assert that there is a link between the follow up of the therapeutic modifications by the general practitioner, whether it's an insertion or an interruption, and an explanation in the summary discharge. However, we notice that 89.5% of therapeutic interruptions and 78.5% of drug insertions are followed by the family care doctor. It is difficult to analyze that excellent rate of follow up because of a lot of biases: monocentric study, lack of comparison according to general practitioners and according to the kind of information transmission.
