Les médicaments des sujets âgés de 75 ans et plus hospitalisés‎ : divergences entre la prescription du médecin traitant et la délivrance du pharmacien

Abstract : Introduction : Iatrogenic medication-related complication particularly affect elderly frail patients with multiple comorbidities. The main objective of this study was to compare information provided by the patient's pharmacist and general practitioner concerning the number of medications and therapeutic classes the patients had been exposed to. Methods : This cross-sectional and prospective study included hospitalised patients aged 75 and over. Their general practitioner and pharmacist were simultaneously contacted and asked to provide a list of all drugs prescribed and delivered in the last 3 months. Results : We obtained information from both the general practitioner and the pharmacist for 11 patients. The number of medicines prescribed and delivered was inconsistent for 10 patients (90,9% ; p=0,04), the number of therapeutic classes differed in 10 patients (90,9%). Inconsistent prescriptions included the following therapeutic classes: cardiovascular drugs (81,8%), antibiotics (45,2%), proton pump inhibitors (27,2%), benzodiazepines (18,2%) and level 2 analgesics (18,2%). It is the pharmacist who provides the information allowing the detection of inconsistent prescriptions in 87,5% of cases (p<0,01). According to two experts asked to compare medication lists provided by the patient's pharmacist and general practitioner in a blinded fashion, particularly the information concerning those medications which have actually been delivered has a clinical impact. Conclusion : Contacting the pharmacist is indispensable in the management of elderly hospitalised patients in order to assure completeness of the patient's prescription.
