Obstacles à la consultation préconceptionnelle en médecine générale‎ : enquête qualitative auprès de 20 femmes en âge de procréer

Abstract : CONTEXT - The prepregnancy visit is recognized as an effective means of assuring maternal and perinatal healthcare. It is recommended by many health authorities, but is not often carried out in France. OBJECTIVES - Main objective : to understand the primary reasons standing in the way of patients carrying out a prepregnancy consultation in general medicine. Secondary objective : To report on the motivations, needs, and interpersonal issues involved and which interfere with achieving this consultation. METHODS - A qualitative study comprised of twenty semi-directed interviews carried out with pregnant women or post birth women in three locations in the Savoie region of France. RESULTS - There is a lack of information about the existence as well as the benefit of this consultation. Women don't feel the need, not feel concerned, or are confused as to the role of the doctor in such care. There is fear to bring up the subject or to see a doctor just for the simple desire to become pregnant. The relationship between the doctor and the patient plays a major role. There are some organizational obstacles as well. However, the motivations and expectations exist and they are easily evoked. CONCLUSIONS - Patients have expressed their difficulty to consult a medical doctor when they plan to become pregnant. To improve care, it is indispensable to develop ways to both inform patients of the existence of this consultation, as well as the inherent benefits. Moreover a quality relationship between the patient and their doctor is a necessary condition to talking about the subject with the patient, and thus creating patient interest and support of the idea of prepregnancy care.
