Évaluation de techniques ultrasonores innovantes pour le diagnostic non invasif de fibrose chez les patients atteints d'une hépatite chronique B ou C

Abstract : Objectives: Assess the performance for the diagnosis of significant fibrosis in patients with chronic viral hepatitis B or C of the SWE elastography compared with the impulse elastography (Fibroscan ®), the reference test being the liver biopsy. Materials and Methods: Prospective study on 18 months. After obtaining the consent, the inclusion of 212 patients with chronic viral hepatitis B or C is planned. Performing a liver biopsy, quantitative measurement in kPa of liver stiffness (Fibroscan® and SWE elastography). Results: Intermediate analysis on the first 30 patients. There were 43% of significant fibrosis and 57% of fibrosis insignificant. The AUROC for the diagnosis of significant fibrosis by SWE elastography was 0.855. For the threshold value of 7.98 kPa, the specificity was 80% and the sensitivity of 73%, with a false positive rate of 11.5% and a false negative rate of 11.5%. A value < to 6.27 kPa allowed to exclude the diagnosis of significant fibrosis and a value > to 10.75 kPa to confirm the diagnosis of significant fibrosis. The AUROC of Fibroscan ® was 0.852, no significant difference from the SWE elastography (p = 0.98). Conclusion: The SWE elastography has showed encouraging results for the diagnosis of significant fibrosis, similar to Fibroscan ®.It remains to be checked at the end of the study if the SWE elastography may show superiority over Fibroscan ®.
