Déterminants individuels et collectifs de l'infection à virus Chikungunya dans la population réunionnaise

Abstract : Introduction: The island of La Reunion experienced an epidemic of Chikungunya virus infection (CHIKV) with a prevalence rate estimated population of 38.2%. Given the magnitude of this health crisis, identifying the determinants of infection in population was conducted to allow appropriate management actions to protect public. Objectives: The objectives of this study were: (1) Measuring the impact of individual and collective determinants of CHIKV infection in population of Reunion. (2) present the results of a predictive score the level of exposure to the vector. (3) show the heterogeneity of the distribution of the prevalence of the island's territory. Methods: We re-used epidemiological data from cross-sectional survey of seroprevalence SEROCHIK initiated by the Clinical Investigation Center - Epidemiological Clinic. From a sample of 2101 Reunion residents aged fifteen years and over, we proceeded in three different regression models (logistic, binomial and Poisson) in bivariate and multivariate variables individually and collectively, completed the calculation of their attributable fractions. Results: The altitude less than 750 meters, the individual dwelling garden and a history of Chikungunya in the vicinity were the factors most strongly predictive of CHIKV infection in the population of Reunion. The lack of knowledge in the mode of transmission, as well as social factors contributing to the distribution of secondary infection in population. Conclusion: Our study shows that collective determinants play a role. However, the behavioral approach during epidemics should be strengthened to make them more effective information campaigns and prevention against Chikungunya.
